Berrien Community Foundation will reduce readmission rates and improve health outcomes among older adults with multiple chronic conditions by expanding the Interagency Care Teams for High Risk Patients (ICT). ICT will develop a web-based communication tool for clinical tracking, connect patients to services, and assist individuals with Medicare/Medicaid enrollment.
