Past Grants

This project will research and develop technology solutions to provide interoperability between the Region 2 Area Agency on Aging’s Intake and Referral data collection platform and the local Community Information Exchange. Creating interoperability will streamline the screening process and help connect older adults to services in the community much faster through improved communication with community partner agencies and local health care systems.
