Henry Ford Health System will test their Heart Healthy Families Program’s ability to improve health outcomes of Black mothers and infants through remote monitoring during the pre-and post-natal period with the goal of preventing infant and maternal mortality. Patients diagnosed with preeclampsia or hypertensive disorders will receive a Veta Health cellular-enabled blood pressure cuff at a prenatal visit or before discharge from the hospital. After the program is introduced, patients will take their blood pressure daily with results transmitted back to the program team through the Veta Health platform and treatment will be managed remotely. Community members will help to develop and vet the educational materials that will be distributed to patients with the blood pressure cuff and participants will be able to provide feedback throughout the program.
