Past Grants

Lake Superior Hospice will develop a continuum of care for older adults with chronic conditions by bringing together several new and expanded palliative care and hospice programs under one agency umbrella. Lake Superior Hospice will pilot the Journeys program, which provides coordinated supportive care for those who have ‘graduated’ from hospice services, helping them follow these patients without an interruption in nursing, social work and spiritual care support.
