Past Grants

To address the immediate and urgent need of providing greater access to care for hundreds of homebound older adults in Detroit, DAAA will bring the “hospital into the home” providing coordinated and preventative care across health settings to improve or stabilize the health status of these patients. These individuals and their caregivers will also be connected to other social supports and programs.
