Past Grants

The Greater Flint Health Coalition will create a health hub in downtown Flint, providing centralized access to a collection of community-based programs and resources, new and expanded GFHC programs, and a shared community health information exchange. Additionally, GFHC will expand its Community Referral Network platform to integrate clinical healthcare partners with community and social service agencies and streamline service delivery to shared patients or clients.
