Past Grants

To improve health outcomes for people with diet-related chronic conditions, this project will strengthen the Fresh Food Pharmacy model, which combines healthy food access with varying levels of clinical support and health coaching. It will track health impacts, cost savings, and patient experiences to refine the approach, expand access, and build the case for broader Medicaid reimbursement across Michigan.
