Michigan State University, Munson Healthcare, and community partners will develop and implement a program to improve nutrition for older adults with chronic conditions in order to reduce risk for readmission. Seniors will receive meals tailored to their specific nutritional needs after hospital discharge, in addition to nutrition education and referrals to other community services as needed. This program will enhance care coordination among clinical services and community-based programs.
