Past Grants

The Extended Care Post Discharge from Addiction Treatment project will provide long-term support to persons leaving treatment to improve their recovery success rates, reduce costs of recidivism, and demonstrate a cost-effective mechanism to enhance the efficacy of treatment. The aim of this program expansion is to provide the persons served at Personalized Nursing LIGHT House, often homeless, unemployed, and with co-occurring mental health disorders, one full year of support similar to Michigan’s Health Professional Recovery Program model.
