Past Grants

This program will launch a Medical Provider Program that integrates home and community-based services with medical and behavioral health care for older adults, aiming to enhance health outcomes and reduce hospitalizations. Nurse practitioners and Licensed Clinical Social Workers will provide in-home assessments, personalized care plans, and behavioral health counseling, ultimately improving quality of life for participants and potentially reducing health care costs.
