To improve health outcomes for foster youth, this project will scale a health literacy curriculum statewide, starting with a pilot in three regions. Building on a Health Fund-supported planning grant that developed the foster-care-specific curriculum, the project will equip transition-age youth with tools to navigate health systems and will embed the model into child welfare practices for long-term, youth-driven impact.
