To help small community organizations and local food providers offer healthy food programs through Medicaid, this project will pilot a model that manages contracts, reimbursement, and coordination on their behalf. It will create a regional network to connect providers with Medicaid health plans, expand access to local nutrition services, and share a free guide so other regions in Michigan can adopt the model.
