Past Grants

To improve maternal and infant health outcomes for Black and Brown families in Benton Harbor, the Continuum of Care Initiative will deploy Maternal and Infant Health (MIH) Navigators to strengthen care coordination and support. Through partnerships with local health providers and community organizations, navigators will help families navigate prenatal and postpartum care, access resources, and overcome barriers to consistent, culturally responsive care.
