Mel Trotter Ministries is requesting support to complete a comprehensive evaluation of how the Community Partners Medical Clinic (CPMC) model works, whether medical services are delivered to homeless individuals in the community efficiently, and how this can be scaled to respond to the ongoing needs of Kent County’s homeless population. The CPMC was created in the summer of 2020 in partnership with Spectrum Health, Trinity Health St. Mary’s, University of Michigan Health West, Grand Valley State University, and Michigan State University to address a gap in health services after the closing of Heartside Clinic, a primary care center serving the homeless population, by bringing the medical community together to assess and develop solutions.
