This project will improve perinatal outcomes by reducing the rate of spontaneous preterm and low birth weight infants, which are the leading contributing factors for perinatal mortality. The program will provide group prenatal care through the CMU Partners Clinic to approximately 120 individuals in each of 2 years. They will measure the outcomes of this specialized care model and compare to baselines for continuing improvements Anticipated outcomes of this project include improved tobacco cessation rates, breastfeeding initiation rates, return to clinic for post-partum visits, and reduced low birth weight and spontaneous preterm birth among their patients.
