Caring for the Caregiver is modeled after the Age-Friendly Community and Communities for a Lifetime assessment tools. The tool will measure how well the health and social service systems in the state’s municipalities and counties are supporting informal caregivers. Domains to be assessed are: 1) In-Home Services & Respite; 2) Care Management Support; 3) Caregiver Training, Education, and Social/Emotional Supports; 4) Safe/Affordable/Accessible Housing; 5) Financial & Legal Information; 6) Transportation; and 7) Healthcare Services. Community stakeholders and advocates, such as human services collaborative bodies, will be able to use the assessment results to advocate for changes at a municipality or county level.
