This project supported the creation of an integrated care team to deliver primary medical care and behavioral health at Isabella Citizens for Health (ICH), a designated Federally Qualified Health Center (FQHC) in Mt. Pleasant. Through the project, ICH added and co-located behavioral health services within the health center. This allowed patients to see both a medical and behavioral health specialist with a single trip or visit to the health center, and the medical provider and behavioral health specialist to discuss the patient’s needs and co-develop the treatment and care plan.
