This project will identify older adults who are socially isolated or lonely by the pharmacy care team they frequently access and trust. Electronic point of care referrals from pharmacies in the four target counties will connect patients to services already accessible and sustained through county councils or commissions on aging (COA), area agencies on aging (AAA), or other service providers in the aging network.
