In Grand Rapids’ Boston Square neighborhood, a full-time navigator will be placed at HUB 07, a new community space that brings health and social services together, to support residents facing food insecurity, housing instability, behavioral health needs, and gaps in early childhood services. The navigator will provide personalized assessments, referrals, and follow-up support to help families access and stay connected to care.
