This proposal aims to address the pressing issue of falls among older adults by creating a centralized, easily accessible hub via 2-1-1 to connect them with fall prevention resources in Michigan. The initiative seeks to enhance awareness of existing fall prevention programs, identify gaps in services, validate a regional tool for program direction, and ensure widespread knowledge of the 2-1-1 hub among key stakeholders to facilitate access to evidence-based fall prevention programs.
