To reduce food insecurity and improve access to nutrition services for transition-age foster youth in Detroit, this project will connect 150 young people to Medicaid benefits and community-based nutrition programs. It will embed Medicaid navigators, provide nutrition education, and train peer mentors to guide other foster youth, creating a model that can be replicated across Michigan.
