Past Grants

This initiative aims to address issues of access to quality and equitable medical care for older adults with chronic disease conditions through a home-based primary care program. The proposal has two related components—the refinement and expansion of the house call model, and the development of an Institutional Equivalent Special Needs Plan (IE-SNP).
