The Health Care and Food Bank Partnership Initiative will strengthen the connection between health care and good nutrition by increasing access to healthy food for individuals struggling with food insecurity and improving health care providers’ awareness of the hunger and nutrition challenges facing low-income patients. Food Gatherers will partner with Michigan Medicine and IHA primary care clinics to identify food insecure patients, refer them to food pantries and community resources, and create an opportunity for patients to discuss food security, nutrition, and health with trusted health care providers.
