This project will expand Hurley Medical Center’s Food FARMacy, a newly established program that addresses food insecurity in Hurley’s patient population by increasing access to healthy foods and assistance programs, like SNAP, WIC, and Double Up Food Bucks. Upon discharge from an inpatient unit or outpatient primary care clinic, an individual receives a referral for nutritious food and can elect to meet with a registered dietitian. The expanded program will reach more pediatric and older adult patients by partnering with other primary care clinics in Genesee County.
