Past Grants

Habitat for Humanity of Kent County (Habitat Kent) will leverage community partnerships to improve health and wellness services through a neighborhood-driven revitalization effort called Plaza Roosevelt. Habitat Kent will use this grant to measure the effectiveness of their convener role and process, as well as the resulting health outcomes of this collaborative neighborhood revitalization model.
