In the 2020 Nutrition & Healthy Lifestyles cohort, the Health Fund awarded a pilot grant to the Food BankCouncil of Michigan in which patients of Grace Health Federally-Qualified Health Center (FQHC) were offered a free, bi-weekly grocery box and one-on-one coaching to those who screened food insecure and were managing a chronic illness. Phase 2 of this initiative will refine FBCM’s model for fresh food access in FQHCs through a sustainable funding strategy for food as medicine in conjunction with the Michigan Department of Health and Human Services (MDHHS), the Michigan Association of Health Plans (MAHP), and the Michigan Primary CareAssociation (MPCA). The Fresh Food Pharmacy model addresses food insecurity as a social determinant of health with a comprehensive grocery and healthy lifestyles program within a FQHC.
