The Senior Health and Housing Initiative will offer a personalized, comprehensive, and coordinated way to assess and address the social and physical needs of older adults living in Detroit’s high-rise apartments. These older adults, most of whom are low-income with limited physical mobility, are often socially isolated without access to on-site care. Through an in-person survey and assessment, project partners will better understand how to address the needs of these older adults and will be able to work with them to develop building-specific interventions and programming, such as on-site transportation. This proposal will support the assessment and program plan design for each building, which will be carried out by the 15 community-based organizations engaged in this collaborative.
