This project will develop the infrastructure for a community integrated network of home nutrition providers, first in Washtenaw County and then across Southeast Michigan. It will leverage additional funding from the Ann Arbor Area Community Foundation to develop the Home Nutrition+ program that provides medically tailored meals (modeled after Food is Medicine), social determinants of health assessments, and referrals to community-based partners to meet other needs. This initiative will coordinate service delivery with uniform practices for recruitment, referral, enrollment, marketing, quality assurance, and evaluation. They will engage older adults in the process through focus groups, one-on-one interviews and through input from the Friendly Caller program.
