Past Grants

This project will support home-based transitional care to transition older veterans who have been discharged from the hospital and have declined in physical function to a more physically active lifestyle in order to reduce readmission rates. The grant will support home care in the post-hospitalization period, providing a VA-home care Link Team to coordinate communication and care between the VA Ann Arbor Healthcare System and home care agencies, provide support for veterans and caregivers, and coach the veteran to optimize functional mobility and physical activity.
