This project addresses the gap between seriously ill cancer patients’ goals and priorities and the care they receive, particularly near the end of life. By developing a person-specific plan from the very beginning of an individual’s care, the approach will better support both patients and families. Medical and nonmedical goals will be documented in a web-based tool for easier reference during care.
