Michigan Oral Health Coalition will develop a cost-effective integration model to increase oral health literacy and access to oral health care services among low-income seniors. Staff from Cherry Hill, a federally qualified health center, will provide oral health education, preventive oral health care services, and a dental home referral to seniors seeking treatment, and will screen seniors for chronic diseases.
