Past Grants

To achieve statewide integration between Michigan’s Area Agencies on Aging and physical health providers, this project will develop and implement best practices for data-driven, integrated care coordination for older adults with complex care needs. Additionally, the project team will recommend a coordinated statewide approach to identify social determinants of health among program participants and connect those individuals to appropriate services.
