Connected2Care will integrate electronic health notifications from health systems and facilities to the Are Agencies on AGing service delivery system to promote care coordination and align physical health and community-based support services. This integration will improve data quality, as well as the efficiency and effectiveness of long-term supports and services (LTSS) care planning.
