This pilot program will create a replicable, sustainable model for Michigan Federal Qualified Health Centers (FQHCs) to integrate food prescription programs into their slate of comprehensive services. Food Bank Council of Michigan will use results of the pilot program to educate and inform its networks of food banks and FQHCs throughout the state, as well as provide evidence to stakeholders for the potential for incentivization of this model through the Medicaid contract or an 1115 Waiver.
