This project will address health inequities for older adults in Michigan rural and urban communities associated with limited access to quality health care, hypertension treatment, healthy food access, and physical activity. This project builds on the American Heart Association’s 2019 Nutrition and Healthy Lifestyles grant EmPowered Clinicians: Improving Hypertension Through Clinical Quality Improvement and Lifestyle Change, which used evidence-based initiatives to improve hypertension control rates. The new grant will expand the initiative to ten FQHCs.
