This project allows jails and external payers and providers to share and integrate treatment information for justice-involved individuals, leading to more effective collaboration, improved care coordination, and increased jail safety. Jails will pilot Care Connect 360, a data portal already used by the Community Mental Health system, which will allow jails to immediately identify the medical and behavioral health needs of incoming justice-involved patients and relay information to the Community Mental Health system to promote seamless transitions to and from the community.
